I-694, Notice of Appeal of Decision Under Sections 245A or 210 of the Immigration and Nationality Act
Use this form to notify USCIS that an alien who was denied permanent residence, temporary residence or a waiver of grounds of inadmissibility under the amnesty program provisions of the Immigration Reform and Control Act of 1986 (IRCA) is appealing the decision to the USCIS Administrative Appeals Office.

Mail this form to the address given on the Notice of Denial.
You may pay the fee with a money order, personal check, cashier’s check, or by credit card using Form G-1450, Authorization for Credit Card Transactions. If you pay by check, you must make your check payable to the U.S. Department of Homeland Security.
