Update to Form G-884, Request for the Return of Original Documents. New Edition Dated 12/02/21.

December 28, 2021

Edition Date: 12/02/21. Starting Feb. 28, 2022, we will only accept the 12/02/21 edition. Until then, you can use the 10/21/19 edition. You can find the edition date at the bottom of the page on the form and instructions.

Edition Date: 09/13/21. Medical examinations signed by the civil surgeon Oct. 1, 2021, or later must use the 09/13/21 edition. We will continue to accept prior editions for medical examinations signed by the civil surgeon before Oct. 1, 2021.
